一种舒适的治疗老年良性阵发性位置性眩晕患者的头位自主训练方法*

摘要：目的探讨一种舒适的头位自主训练方法治疗老年良性阵发性位置性眩晕的疗效。方法选取本院神经内科2020年1月—2022年6月住院的老年良性阵发性位置性眩晕患者164例为研究对象，按入院顺序随机分成实验组及对照组，每组82例。对照组给予常规药物治疗，实验组在常规药物治疗基础上加用头位自主训练，评估临床治疗有效率。结果实验组3次头位自主训练的有效率及治愈率均高于对照组（χ2＝51.19，59.27，56.23，40.83，47.06，49.61，均P＜0.01）。结论老年良性阵发性位置性眩晕的常规复位治疗可适当等待，避免复位过程中带来灾难性后果，等待过程中可采取头位自主训练治疗。

Abstract：Objective To investigate the therapeutic effect of a comfortable head-position self-training method on benign paroxysmal positional vertigo （BPPV） in the elderly patients. Methods A total of 164 elderly patients with BPPV admitted to the department of neurology in our hospital from January 2020 to June 2022 were selected as the research subjects and randomly divided into the experimental group and the control group according to the order of admission， with 82 cases in each group. The control group received conventional drug treatment， while the experimental group received conventional drug treatment combined with head-position self-training. The clinical treatment efficacy was evaluated. Results The efficacy and cure rates of the three sessions of head-position self-training in the experimental group were significantly higher than those in the control group （χ2＝51.19，59.27，56.23，40.83，47.06，49.61，all P＜0.01）. Conclusion Conventional repositioning therapy for elderly BPPV can be appropriately delayed to avoid catastrophic consequences during repositioning. During the waiting period， head-position self-training can be used as a treatment option.
