系统性红斑狼疮患者外周血NKG2D和干扰素-α的表达及意义*

 摘要:目的 探讨系统性红斑狼疮(systemic lupus erythematosu,SLE)患者中外周血NKG2D和干扰素-α(IFN-α)的表达及临床意义。方法 选取2021年1月—2022年6月内蒙古包钢医院30例初发SLE患者(病例组)和30例健康志愿者(健康组),比较两组外周血IFN-α水平、自然杀伤细胞(NK细胞)绝对数、NKG2D表达差异及病例组治疗前后相关指标变化,分析其中部分指标与血红蛋白、补体C3、(SLE疾病活动指数)SLEDAI评分的相关性。结果 与健康组相比,病例组外周血NK细胞比例、NK细胞绝对数及NKG2D表达均显著降低,组间比较差异均有统计学意义(t=2.703、2.517、3.321,均P<0.05);治疗后,病例组NK细胞比例、NK细胞绝对数及NKG2D表达均显著上调,治疗前后比较差异均有统计学意义(t=0.117、0.062、0.058,均P<0.05)。NK细胞绝对数和NKG2D与血红蛋白、补体C3均呈正相关,与SLEDAI评分均呈负相关(r=0.457、0.557、0.351、0.312、-0.387、-0.398,均P<0.05)。IFN-α水平与NK细胞绝对数及NKG2D均呈负相关(r=-0.243、0.256,均P<0.05)。结论 病例组NK细胞水平及功能异常,NKG2D表达异常可能通过影响浆细胞样树突状细胞(plasmacytoid dendritic cell,PDC)分泌IFN-α,从而参与了SLE的发病及系统受累。

 Abstract:Objective To investigate the expression of NKG2D and interferon-α (IFN-α) in peripheral blood of patients with systemic lupus erythematosus (SLE)and to evaluate their clinical significance.  Methods Thirty newly diagnosed SLE patients(case group)and 30 healthy volunteers(healthy group)were enrolled from Inner Mongolia Baogang Hospital  between January 2021 and June 2022. Peripheral blood levels of IFN-α, absolute count of natural killer(NK)cells, and NKG2D expression were measured and compared between groups. Changes in relevant indicators before and after treatment were evaluated in the case group. Correlations between certain indicators and hemoglobin levels, complement C3 levels, and SLE Disease Activity Index(SLEDAI)score were analyzed. Results Compared with the healthy group, peripheral blood NK cell proportion, absolute NK cell count, and NKG2D expression in the case group were significantly decreased, the differences between groups were statistically significant (t=2.703、2.517、3.321, all P<0.05). After treatment, NK cell proportion, absolute count, and NKG2D expression in the case group were significantly increased, there were statistically significant differences before and after treatment(t=0.117、0.062、0.058, all P<0.05).Absolute NK cell count and NKG2D expression showed positive correlations with hemoglobin and complement C3 levels, and negative correlations with SLEDAI score(r=0.457、0.557、0.351、0.312、-0.387、-0.398, all P<0.05). In addition, IFN-α levels were negatively correlated with both absolute NK cell count and NKG2D expression(r=-0.243、0.256,all P<0.05). Conclusion The case group exhibited abnormal NK cell levels and function. Abnormal NKG2D expression may participate in SLE pathogenesis and systemic involvement by influencing plasmacytoid dendritic cell (PDC)secretion of IFN-α.
