耳穴揿针辅助治疗对合并不安腿综合征的血液透析患者睡眠质量及血清铁调素的影响

摘要：目的观察耳穴揿针辅助治疗对血液透析患者不安腿综合征的疗效。方法采用目的抽样法，选取2023年3月—2024年6月在上海市杨浦区市东医院透析中心治疗的90例伴不安腿综合征的血液透析患者，依据患者意愿分为揿针组（41例）和对照组（49例）。对照组予以左旋多巴常规治疗，揿针组在对照组基础上给予耳穴揿针辅助治疗。比较两组的一般资料、国际不安腿综合征评估量表（IRLS）评分、匹兹堡睡眠质量指数（PSQI）分值及血清铁调素水平。结果治疗6周后，揿针组患者的IRLS总分、睡眠时间分值、睡眠质量分值、PSQI总分均较对照组低，差值分别为－4.620（95%CI：－7.075～－2.164）、0.601（95%CI：0.432～0.788）、－0.290（95%CI：－0.421～－0.159）、－1.050（95%CI：－1.758～－0.342），差异均有统计学意义（均P＜0.05）；两组血清铁调素比较差异无统计学意义（P＞0.05）。结论耳穴揿针辅助治疗可减轻血液透析患者不安腿综合征的症状，并改善其睡眠质量，但对血清铁调素无明显影响。

Abstract：Objective To observe the therapeutic effect of auricular acupuncture on restless legs syndrome in hemodialysis patients. Methods A total of 90 hemodialysis patients with restless leg syndrome who were treated in the dialysis center of Shanghai Yangpu District Shidong Hospital from March 2023 to June 2024 were selected as the research objects by purposive sampling method. According to the wishes of the patients, they were divided into thumbtack needle group （n＝41） and control group （n＝49）. The control group was given routine treatment of levodopa. On this basis, the thumbtack needle group was given auricular thumbtack needle adjuvant therapy. The general data, total score of International Restless Legs Syndrome Assessment Scale （IRLS）, Pittsburgh Sleep Quality Index （PSQI） score and serum hepcidin level were compared between the two groups. Results After 6 weeks of treatment, the thumbtack needle group had lower IRLS total scores, sleep duration scores, sleep quality scores, and PSQI total scores compared with the control group, with differences of －4.620 （95% CI: －7.075 to －2.164）, 0.601 （95% CI: 0.432 to 0.788）, －0.290 （95% CI: －0.421 to －0.159）, and －1.050 （95% CI: －1.758 to －0.342）, respectively. All differences were statistically significant （all P＜0.05）. No statistically significant difference in serum hepcidin levels was observed between the two groups （P＞0.05）. Conclusion Auricular acupuncture therapy can alleviate restless leg syndrome symptoms and improve sleep quality in hemodialysis patients, but has no significant effect on serum hepcidin.
