2020 —2022年贺州地区中医医院和综合医院三代头孢菌素类使用量与大肠埃希菌耐药率的相关性研究*

摘要：目的分析2020 —2022年贺州地区中医医院和综合医院第三代头孢菌素类抗菌药（third-generation cephalosporin，TGC）使用量与大肠埃希菌（Escherichia coli，E.coli）耐药率的相关性。方法将2020 年1 月—2022 年12 月贺州市中医医院和贺州市人民医院各科送检临床微生物标本中检出的E.coli作为菌株来源，共3 139株。其中，将贺州市中医医院1 020株作为中医医院组，贺州市人民医院共2 119株作为综合医院组。统计分析两组TGC使用量［抗菌药物的用药频度（DDDs）和使用强度］和E.coli耐药率，并进行相关性分析。结果两组TGC的E.coli耐药率均无显著性差异（χ2＝0.065、0.345、0.152，均P＞0.05）；两组TGC的DDDs与E.coli耐药率均无显著相关性（r＝－0.827、0.995，均P＞0.05）。中医医院组TGC的使用强度与E.coli的耐药率无显著相关性（r＝－0.542，P＝0.635）；综合医院组抗菌药物的使用强度与E.coli耐药率呈正相关（r＝0.999，P＝0.011），敏感性分析显示结果具有稳定性。结论贺州地区中医医院TGC使用度（DDDs和使用强度）与E.coli耐药率无相关性，综合医院使用强度与E.coli耐药率呈正相关。

Abstract：Objective To analyze the correlation between the usage of third-generation cephalosporins （TGC） and the drug resistance rate of Escherichia coli （E.coli） in Traditional Chinese Medicine （TCM） Hospitals and General Hospitals in Hezhou area from 2020 to 2022. Methods Clinical microbiological specimens from various departments of Hezhou TCM Hospital and Hezhou People's Hospital, collected from January 2020 to December 2022, were used for the study. A total of 3 139 E. coli strains were isolated, with 1 020 strains from Hezhou TCM Hospital （TCM hospital group） and 2 119 strains from Hezhou People's Hospital （general hospital group）. The usage of TGC was measured by the drug use frequency （DDDs） and intensity, and the E. coli resistance rate was calculated for both groups. Correlation analysis was performed. Results There was no significant difference in the E. coli resistance rate to TGC between the two groups （χ2＝0.065, 0.345, 0.152; all P＞0.05）. There was no significant correlation between the DDDs of TGC and the E. coli resistance rate in both groups （r＝－0.827, 0.995; both P＞0.05）. In the TCM hospital group, the intensity of TGC use was not significantly correlated with the E. coli resistance rate （r＝－0.542, P＝0.635）. In the general hospital group, the intensity of antibiotic use showed a positive correlation with the E. coli resistance rate （r＝0.999, P＝0.011）, and sensitivity analysis confirmed the stability of the result. Conclusion There was no correlation between the use of TGC （DDDs and use intensity） and the drug resistance rate of E.coli in Hezhou TCM Hospitals. In contrast, in General Hospitals, the intensity of TGC use is positively correlated with the E. coli resistance rate.
